New Accident Reporting Requirements
Motorists involved in collisions resulting in injury, death or damage of $1,000 or more (as determined by the reporting officer) must file an "INDIANA OPERATOR'S PROOF OF INSURANCE/CRASH REPORT" (SR 21 - State Form 52441) within 10 days. This form must be filed with the BUREAU OF MOTOR VEHICLES. Your insurance agent must sign the form if you had insurance at the time of the accident.
This policy differs from past requirements, wherein motorists filed an accident report (Yellow Form) with the Indiana State Police. The Yellow form has been replaced with this current SR 21.
